Does Shopify Store-to-Store Sync automatically create metafield definitions?

Yes, it can do that. It compares metafield definitions from the main store to sub-stores and automatically creates any missing metafields in the sub-stores within the Shopify admin.

However, you still need to map the metafield in the sub-store and enable the required options which is auto create metafield definition if not found, as it only creates the metafield in Shopify admin.
